TOPIC: RIDEM PROCESS IMPROVEMENT NEW FORM and APPENDIX A Checklist

The next phase of implementation of the new single application form for Stormwater Construction and Water Quality Certification is going live January 2nd.  Instructions and the forms are available on the website at  http://www.dem.ri.gov/programs/water/permits/ripdes/stormwater/

Please note that this is a "smart-form" so you must fill out the form on the computer and then print it out for signatures.  Because it is a smart form, fees, submittals, permit numbers all change when certain fields are filled in.
