[Owtsinfo] Attention OWTS Professionals: OWTS Permitting Forms Have Been Updated to Reflect Revised Rule Numbers

Effective November 25, 2018 the OWTS Rules were revised and corresponding OWTS rule numbers have changed. This required the OWTS Program to update some of the permitting forms which are available on our website.

Please use the updated versions going forward. Here is the list of forms which were recently updated:



  1.  Site Evaluation Form
  2.  System Suitability Determination - Residential/Commercial
  3.  Variance Request
  4.  Joint OWTS/Wetlands Permit Application Form
  5.  Repair Submission Checklist
  6.  Affidavit of Continuing Validity of Field Data
