<li class="MsoListParagraph" style="mso-list:l0 level1 lfo1"><span style="font-family:"Times New Roman",serif">A Renewal Application Form (individuals who also hold a Cl-IV license were mailed two Renewal Application Forms),<o:p></o:p></span></li><li class="MsoListParagraph" style="mso-list:l0 level1 lfo1"><span style="font-family:"Times New Roman",serif">A list of outstanding Certificates of Construction (COCs) and As-Builts (ASBs) associated with their license reported due between 1/1/16 (the start
of this three-year cycle) and 10/10/18, if applicable,<o:p></o:p></span></li><li class="MsoListParagraph" style="margin-bottom:6.0pt;mso-list:l0 level1 lfo1">
<span style="font-family:"Times New Roman",serif">A transmittal memo.<o:p></o:p></span></li></ul>
<p class="MsoNormal" style="margin-bottom:6.0pt"><span style="font-family:"Times New Roman",serif">Please note that Class II and III (and Cl-IV licenses held by individuals with a Cl-II or III license) will expire 12/31/18.<o:p></o:p></span></p>
<p class="MsoNormal" style="margin-bottom:6.0pt"><b><span style="font-family:"Times New Roman",serif">OWTS rule 11.1 prohibits an individual from performing licensed activities with an expired license</span></b><span style="font-family:"Times New Roman",serif">.
Renewal is required before 1/2/19 to maintain active license status and authority to perform activities authorized by your license. If you have installations in progress, anticipate beginning construction before December 31, 2018 or shortly thereafter, or
plan to perform soil evaluations after December 31, 2018, be aware that <b>expiration of your license will prohibit you from performing licensed activities including calling in a Start of Construction and performing soil evaluations</b>.<o:p></o:p></span></p>
