[Owtsinfo] Reminder to OWTS Designers Submitting Repair Applications to DEM

[owtsinfo at listserve.ri.gov]

Effective October 1, 2018 all OWTS Applications for Repair must be accompanied by the attached Repair Submission Checklist. In addition, all the information outlined on the checklist must be addressed in the repair application.

Please complete all sections of the form, if a section does not apply, please indicate this by writing "Not Applicable or N/A". Repair applications that are incomplete and that do not include this form will be considered unacceptable and will be returned administratively to the designer for editing and resubmission.

Your diligence in using this form allows DEM staff to complete the application reviews smoothly and quickly which in the end will benefit the professional community, the clients you serve, and our environment.
